About the Media Consent Forms
The Freedom Fund takes the safeguarding of both the people we serve and our own staff incredibly seriously. We have established systems to ensure that power isn’t being abused and vulnerability isn’t exploited. As a part of these efforts, we require a media release form to be used with any individual who is being asked to participate in photo, video or testimonial projects. Staff are trained on the use of these forms to ensure that express consent is provided and that participants are aware that they can withdraw consent at any time.
Included in this download are two media release forms, one for adults and one for minors.
